You log in with the username and SSH key you choose when creating the VPS, and that user has passwordless sudo.
Usually within a minute. On first boot, cloud-init creates your user with your SSH key, sets the hostname and configures your IPv4 and IPv6 addresses. Then you can log in over SSH.
In our own hardware in Enschede, the Netherlands, connected to Amsterdam over our private backbone. Your data stays in the Netherlands.
Every VPS has a web console in the dash, so you can always get back in, even if SSH or the network config is broken.
